Additional Comments
Spoke to customer about incident and inspected unit. Found forks bent, mast
misaligned, tilt under rod slightly bent, side shift cylinder leaking from gland nut
seal, over head guard with slight bend, and body damage to left side of unit. Test
drove unit and it had a noticeable wobble during travel on level ground. Unit would
need to go the shop to be further evaluated by remove mast from drive axle.
